What to Expect
During your first visit, your nurse will:
- Review your medical history and current health concerns
- Assess your home environment for safety
- Discuss your care goals and preferences
- Create an initial care plan and schedule follow-up visits
You'll have the opportunity to ask questions and discuss your care options.
What to Have Ready
To help your nurse prepare your care plan effectively, please have:
- Your DVA Gold or White Card
- List of medications (including over-the-counter and supplements)
- GP and specialist contact details
- Hospital discharge summary, if applicable
- Emergency contact information
Creating a Safe and Comfortable Environment
- Ensure clear access to the area where care will be provided.
- Keep pets safely in another room during the visit.
- Have good lighting and a comfortable chair for discussion.
