Each client receives an individualised care plan based on their health needs.
Before your first appointment, please have:
Ask your GP, hospital, or DVA delegate to submit a referral.
The referral is sent directly to Core Healthcare Services.
Our nurse will contact you to schedule your first visit and assessment.
We create a care plan tailored to your needs.
All services for eligible clients are fully funded by the DVA — no out-of-pocket costs.
We provide respectful, professional, and holistic nursing care that enhances quality of life and supports independence.
