DVA Community Nursing Services Guide

Purpose of This Guide

This guide explains how eligible veterans, war widows, and widowers can access DVA-funded Community Nursing Services.

What to Expect

The DVA Community Nursing Program provides professional nursing care delivered in your own home by qualified nurses.

Our services include:

  • Medication management and education
  • Wound care and recovery support
  • Chronic disease monitoring
  • Palliative and post-hospital care
  • Hygiene and mobility assistance

Each client receives an individualised care plan based on their health needs.

Information Checklist

Before your first appointment, please have:

  • DVA Gold or White Card
  • Current medications and dosage details
  • GP and specialist contact details
  • Any recent hospital discharge summary
  • List of allergies or ongoing conditions

How to Access DVA-Funded Care

Ask your GP, hospital, or DVA delegate to submit a referral.

The referral is sent directly to Core Healthcare Services.

Our nurse will contact you to schedule your first visit and assessment.

We create a care plan tailored to your needs.

All services for eligible clients are fully funded by the DVA — no out-of-pocket costs.
