DVA Community Nursing Program
The Department of Veterans' Affairs (DVA) Community Nursing Program helps eligible veterans and war widows/widowers receive professional nursing care in their own homes.
At Core Healthcare Services, we proudly deliver DVA-approved nursing to support independence, safety, and wellbeing.
Who Is Eligible
You may be eligible if you hold a DVA Gold Card or a White Card for an accepted service-related condition.
Our care coordination team can confirm eligibility and assist with referrals.
Services Provided
Core Healthcare Services provides:
- Medication management and education
- Wound dressing and infection prevention
- Personal care and mobility assistance
- Chronic disease monitoring (e.g., diabetes, heart conditions)
- Palliative and post-hospital support
Each plan is tailored to your needs and reviewed regularly by our nurses.
How to Access Services
Speak with your GP
Contact your GP, hospital, or DVA delegate to discuss your needs.
Complete Referral
Your healthcare provider completes a Community Nursing referral form.
Home Visit
Our nurse will contact you to arrange your first home visit.
DVA-funded services are free for eligible clients.
